2.0 PURPOSE:

2.1 To establish rules to annually license methadone treatment programs in King County.

3.0 ORGANIZATIONS AFFECTED:

The Seattle-King County Department of Public Health - the Division of Alcoholism and Substance Abuse Services, and agencies certified by the State Division of Alcohol and Substance Abuse Services to provide outpatient chemical dependency treatment services.

5.0 DEFINITIONS:

5.1 "Methadone treatment program" means an outpatient clinic certified for methadone treatment by state and federal agencies and licensed by King County.

5.2 "Agency" means a program certified by the State Division of Alcohol and Substance Abuse Services to provide outpatient chemical dependency treatment services.

5.4 "Director" means the director of the Seattle-King County department of public health or the Director's designee.

5.5 "Department" means the Seattle-King County Department of Public Health, the Division of Alcoholism and Substance Abuse Services.

5.6 "License Fee" means a fee charged to a Methadone treatment program by the Seattle-King County Department of Public Health, the Division of Alcoholism and Substance Abuse Services for the service/purpose of carrying out and enforcing statutes, ordinances, or regulations.

6.0 POLICIES:

6.1 Licensed methadone treatment programs shall not have a caseload in excess of three hundred fifty clients.

6.2 The annual fee for a methadone license is $2,500.

6.3 The Director of the Seattle-King County Department of Public Health establishes the application requirements for agencies seeking licensing.

6.4 In order to obtain an annual license from King County an agency must be certified by the State Division of Alcohol and Substance Abuse Services to provide outpatient chemical dependency treatment services. In addition, an agency shall:

6.4.1 Submit to the department a copy of registration with the Washington State Board of Pharmacy.

6.4.2 Submit to the department a copy of the application to the Federal Drug Enforcement Administration.

6.4.3 Submit to the department a copy of the application to the Federal Food and Drug Administration.

6.4.4 Complete all license application requirements established by the Director.

6.4.5 Pay the annual license fee.

6.5 In order for the department to renew an annual license of a methadone treatment program, the agency shall:

6.5.1 Maintain certification from the State Division of Alcohol and Substance Abuse Services to provide outpatient chemical dependency treatment services.

6.5.2 Substantially meet all opiate dependency treatment requirements specified in WAC 440-22-500.

6.5.3 Complete all license renewal requirements.

6.5.4 Pay the annual license renewal fee.

7.0 PROCEDURES:

REQUEST FOR NEW METHADONE LICENSE FOR AGENCIES NOT PROVIDING METHADONE TREATMENT SERVICES:

7.5 Pays the annual license fee of $2,500.

7.15 Pay the annual license fee of $2,500.

7.17 Reviews application criteria.

7.18 Collects annual license fee.

7.19 Issues the County methadone license.

ANNUAL RENEWAL OF METHADONE LICENSE:

7.20 Meets all opiate dependency treatment requirements specified in WAC 440-22-500.

7.21 Completes all license renewal requirements established by the Director.

7.22 Pays the annual license renewal fee of $2,500.

7.23 Determines if an agency is certified by the State Division of Alcohol and Substance Abuse Services to provide outpatient chemical dependency treatment services.

7.24 Determines if an agency meets all opiate dependency treatment requirements specified in WAC 440-22-500.

7.25 Establishes and reviews license renewal requirements.

7.26 Collects the annual license fee.

8.0 RESPONSIBILITIES:

8.1 The agency is responsible for submitting to the department all forms required by this document, completing all license application requirements established by the Director, and payment of the annual license fee. For renewal, the agency is responsible for maintaining certification from the State Division of Alcohol and Substance Abuse Services to provide outpatient chemical dependency treatment services, meeting all opiate dependency treatment requirements specified in WAC 440-22-500, completing all license renewal requirements established by the Director, and payment of the annual license fee.

8.2 The department reviews the submitted forms or requirements and collects the annual license fee.

For renewal, the department determines if the agency maintains its certification from the State Division of Alcohol and Substance Abuse Services, meets all opiate dependency treatment requirements specified in WAC 440-22-500, completes all license renewal requirements and collects the annual license fee.
