Health Care for the Homeless Network
The Health Care for the Homeless Network (HCHN) provides health care services for people experiencing homelessness in King County and provides leadership to help change the conditions that deprive our neighbors of home and health.
Street Medicine Program: Medical outreach for people experiencing homelessness
The street medicine model promises to improve our capacity to flexibly and meaningfully serve those who are the most in need of outreach and health services – bringing care to those who are often unable to make it to scheduled appointments or locations.
Kids Plus
Kids Plus is a multidisciplinary team of nurses and social workers, addressing the health and social needs of children and families experiencing homelessness. We specialize in medical case management, yet we work to address the many complex needs that impact the health and well-being of children.
